How to Add a Habit Tracker to Your Custom Planner
Build the daily habits that actually stick, with pages designed for exactly that.
If you've ever Googled "habit tracker" or "habit diary," there's a good chance you already know the feeling. You want to build better daily routines, but nothing you've tried has quite stuck. A habit journal or habit tracker is one of the most powerful tools for changing behaviour, and the research backs it up.
But here's the thing: a generic habit tracking template rarely works long-term, because it wasn't designed for you. At Your Perfect Planner, we believe your planner should flex around your life, not the other way around. That's why we offer two dedicated habit tracker pages you can add directly to your custom planner.
Our habit tracker pages are fully customisable add-ons. You choose exactly which pages go into your planner when you build it. No filler, no fluff, just the pages you'll actually use.
Why a Habit Tracker Actually Works
Habit tracking works because it makes progress visible. When you physically tick off or fill in a habit each day, you create a chain, and the simple act of not wanting to break that chain becomes surprisingly motivating. It's the foundation of habit-stacking systems used by everyone from elite athletes to productivity coaches.
A dedicated habit diary page in your planner means your tracking lives alongside your schedule, your goals, and your weekly planning. Not buried in an app you forget to open, or on a random sticky note that gets lost by Thursday.
- Visual progress seeing your streak motivates you to keep going
- Accountability a physical record is harder to ignore than a digital one
- Everything in one place your habits sit right next to your weekly schedule
- Reflection monthly habit journals let you spot patterns and adjust
Our Two Habit Tracker Pages
We've designed two different habit tracking pages to suit different planning styles. Whether you want to track multiple habits across a whole month, or go deep on one habit across an entire year, there's an option for you.
Track multiple habits on the one page across the entire month. Perfect for building a well-rounded daily routine covering exercise, water, sleep, journalling, and more.
Dedicate an entire page to tracking one single habit across the whole year. Great for a big goal you want to stay accountable to, like daily movement, reading, or a screen-free morning.
How to Add a Habit Tracker to Your Planner
Adding a habit tracker page to your custom planner is simple. Here's exactly how it works:
Choose your planner format
Start by selecting the planning style that suits how you organise your days. Not sure which one? Daily planners give you a full page per day, weekly planners spread your week across one to two pages.
Build your planner page by page
Once you've selected your planner format, you'll move into the page selection stage. This is where you choose every single page that goes inside your planner.
Find and add your habit tracker pages
Yearly habit trackers are available to add at the start of your planner. Monthly habit trackers are available to add in between each month, or grouped together at the end. You can add one type or both!
Complete your order and that's it!
Your planner is printed and shipped to you, fully assembled with your habit tracker pages right where you need them.

What Habits Can You Track?
This is where it gets fun. Our habit tracker pages are completely blank, so you fill in the habits that matter to you, not a predetermined list that may not match your life at all. Here are some ideas to get you started:
Popular habits our customers track:
- Drinking 2L of water daily
- Exercise or daily movement
- Reading for 20 or more minutes
- Meditation or breathwork
- Journalling or a gratitude practice
- Getting to bed before 10:30pm
- Screen-free mornings
- Taking vitamins or supplements

Tips for Sticking to Your Habit Tracker
A beautiful habit journal is only as good as the habits you build around using it. Here's what actually works:
Start with 3 to 5 habits, not 20
The most common mistake is tracking too many things at once. Start with three to five habits maximum and build from there once they feel automatic.
Fill it in at the same time each day
Pair your habit tracking with an existing routine, like your morning coffee, evening wind-down, or daily planner check-in. Consistency in when you track helps the habit of tracking itself stick.
Don't aim for perfection
Missing a day doesn't mean you've failed. The key rule is never miss twice. One missed day is fine, two in a row is where habits tend to break down.
Keep your planner somewhere visible
Out of sight, out of mind. Leave your planner open on your desk or kitchen bench so your habit tracker is always in view.
